REQUESTING A MAIL-IN BALLOT

  1. Fill out an online request form on your county Supervisor of Elections website

     

  2. Print and fill out the request form, then mail or email it to your county Supervisor of Elections

Requests for vote-by-mail ballots must be received in your local Supervisor of Elections office no later than 5:00 pm on the 12th day before an election.
